IMPORTANT NOTICE
Beginning October 1, 2021, all immigrant visa (except K visa) applicants must be fully vaccinated for COVID-19 for the medical examination to be considered complete. Some waivers for the COVID vaccination will still be applicable, including if the vaccine is not age appropriate. Panel physicians will begin screening applicants for age-appropriate COVID-19 vaccinations on October 1, 2021. Medical exams completed prior to that date will not be subject to this requirement, regardless of when the immigrant or K visa is issued and when the traveler enters the United States. Additional information is available from the Centers for Disease Control and Prevention (CDC).
Please also note that:
- All applicants with symptoms of COVID-19 should be tested for COVID-19 and must postpone the immigrant visa medical examination until they have met recovery criteria to end isolation.
- Close contacts of persons with COVID-19 should also be tested and must postpone the medical examination until quarantine criteria have been met.
- Screening testing of asymptomatic applicants for COVID-19 as part of the immigrant medical evaluation may be required at the discretion of the panel physician.
COVID-19 Screening Results and Travel Clearance
All international air travelers to the United States aged 2 years or older must be tested within 3 days of the flight and show documentation of a negative COVID-19 result to the airline before boarding their flight. Applicants who have recovered from COVID-19 will not need to be retested for travel if they travel within 3 months of their positive test and can provide appropriate documentation.
English translations of all foreign language documents are required. Any foreign language document must be accompanied by a full English translation that the translator has certified as complete and correct. The translator must certify that he/she is competent to translate the foreign language into English. Please have the translator’s signature on the bottom of the documents. It is not necessary to have the translation notarized.
Do not send anything to the Embassy unless you are requested to do so. Bring your documents to the interview. Before making an appointment, make sure you have all the necessary documents.

□ | PASSPORT: A passport must be valid for travel to the United States and must have at least six months validity beyond the issuance date of the visa. |
□ | DS-160 CONFIRMATION PAGE: After receiving instructions from the Embassy in Jakarta, each person applying for a visa, regardless of age, must complete and submit the DS-160 Visa Application online. You must print out the application confirmation page and bring it with you to your interview. |
□ | PHOTOGRAPHS: Two identical color 5cm x 5cm photo with a white background, taken within the last six months. Applicants are not permitted to wear eyeglasses in photos. |

APPLICATION FEE: You must pay the appropriate visa application fee. In most cases, each visa applicant, including children, is required to pay a non-refundable, non-transferable Machine-Readable Visa (MRV) application fee, whether a visa is issued or not. Please follow the instructions below for paying your visa application fee. Note: Applicants are only able to reschedule their appointments up to three times. Please plan accordingly so that you are not required to pay another visa application fee. Visa application fees are nonrefundable. Fee Payment Options Electronic Funds Transfer (EFT) You may pay your MRV Application Fee by an electronic transfer from any Indonesian Rupiah bank account in Indonesia. Your bank may apply charges to your account for EFT transactions. If you are applying for multiple visas for your family or group, all of the MRV Application Fees can be paid in one transfer. Please note that the total amount CANNOT exceed payment for MORE THAN 5 applicants.
Cash at Bank (CIMB Niaga) In Indonesia, you may pay your MRV Application Fee with cash at any branches of CIMB Niaga. You must first register on the applicant site and choose the ‘schedule your appointment’ option to access the payment option details. See below for the steps.
NOTE: By paying the MRV Application Fee you acknowledge that this is a non-refundable visa application processing fee required as a prerequisite to the processing of an application. It does not indicate the existence of a contract with the applicant, nor does it guarantee a successful application. There is no refund possible regardless of the outcome of this application. The Indonesian rupiah (IDR) amounts are based on the consular currency exchange rate determined by the U.S. Department of State. Applicants who do not pay the correct fee for their visa type may be restricted from scheduling an interview. Deposit Slips NOTE: If you already have an existing profile and would like to make a payment, you may select and print the deposit slip that matches your visa application fee from below Scheduling Your Interview Cash at Bank (CMB Niaga Bank) You can schedule your interview after 12:00 noon the next business day after date of payment. Whether you schedule your appointment online or contact our call center, you will need the Virtual Account number printed on your receipt. (This is the UID printed on your payment slip when you created your online profile.) Note: Your Virtual Account will be 16 Digits in length, please use the 16-digit Virtual Account to schedule your interview. You also need to have your passport number and your fee payment date in order to schedule your visa interview. EFT Payment You can schedule your interview after 3:00 pm on the first banking day following the date you pay your visa application fee via EFT Whether you schedule your appointment online or contact our call center, you will need the Virtual Account number printed on your receipt. (This is the UID printed on your payment slip when you created your online profile.) Note: Your Virtual Account will be 16 Digits in length, please use the 16-digit Virtual Account to schedule your interview. You also need to have your passport number and your fee payment date in order to schedule your visa interview. □ | BIRTH CERTIFICATES: You will need to provide the original certificate, one photocopy and a translation of the birth certificate for each visa applicant. |

□ | EVIDENCE OF DISSOLUTION OF PRIOR MARRIAGE(S): If you were previously married, you need to provide the original certificate, one photocopy and a translation of divorce decree(s) or death certificate(s). |
□ | AFFIDAVIT OF SUPPORT: Evidence which will show that you and your children, if any, are not likely to become public charges in the United States. The Affidavit of Support (I-134) and supporting documents are valid for one year since the date of signature. |

POLICE CERTIFICATE: Police certificate(s) from your present country of residence for six months or more, and all countries where you have lived for one year since age 16 are required. Police certificates are also required for accompanying children age 16 or older. A police certificate must also be obtained from the police authorities of any place where the applicant has been arrested for any reason, regardless of the length of residence. Police certificates are valid for two years. They must be valid when the visa is issued. Police records from certain countries are considered unobtainable. Please check Reciprocity by Country. □ | COURT AND PRISON RECORDS: Persons convicted of a crime must obtain a certified copy of each court record and any prison record. The records are required, even if the applicant was acquitted or benefitted from an amnesty, pardon or other act of clemency, regardless of whether or not they may have benefited from an amnesty or pardon. This also applies to juvenile offenses. |
□ | MILITARY DISCHARGE RECORDS: A certified copy of any military record must describe the nature of any military service the applicant has performed. |

MEDICAL EXAMINATION: The U.S. Public Health Service requires fiancée visa applicants to have a medical examination at a designated facility. We cannot accept medical examinations completed by a physician in the United States or by the applicant’s own physician. The medical examination forms are with the doctor of designated facilities.
